Parametri diversi, come le dimensioni, la posizione, il tempo di diagnosi della lesione, ma anche le condizioni generali del paziente, hanno un'influenza significativa sull'esito clinico del paziente. Spesso questa situazione è accompagnata da sintomi di sepsi e da un tasso di morbilità significativo con un tasso di mortalità sostanziale corrispondente. Per questo motivo, il trattamento di questa situazione clinica rappresenta spesso una sfida per il medico.
Oltre all'esame chirurgico e al sistema di stent endoscopico, sono state fatte esperienze positive con la terapia Endo-SPONGE® per il tratto gastrointestinale inferiore. Ora è disponibile anche la terapia del vuoto endoluminale per l'apparato gastrointestinale superiore - Eso-SPONGE®.
La perdita anastomotica è una delle complicanze più temute dopo l'esofagectomia, con un'incidenza fino al 30%, [1-10] e un'elevata morbilità e mortalità associate. [1-10] I pazienti con condizioni quali calcificazione delle arterie, insufficienza cardiaca, ipertensione, insufficienza renale, obesità o diabete presentano un rischio maggiore di perdita anastomotica dopo l'esofagectomia. [11], [12], [13] Per questi pazienti esiste la possibilità di utilizzare Eso-SPONGE® per prevenire lo sviluppo di perdite anastomotiche.
*Il tratto GI superiore si riferisce all'esofago, allo stomaco e al duodeno, accessibili per via endoscopica all'interno dell'intervallo della lunghezza dell'overtube.
Il principio di base della terapia sottovuoto Eso-SPONGE® consiste nell'inserimento di una spugna a pori aperti nella cavità di dispersione o nell'area endoluminale o nel difetto utilizzando un endoscopio flessibile. Un tubo di drenaggio collegato alla spugna viene fatto passare attraverso il naso e collegato a una pompa a basso vuoto. Mediante l'applicazione di pressione negativa, si ottiene un drenaggio continuo efficace ed è possibile evitare l'accumulo di secrezioni. La spugna induce la pulizia dei rivestimenti di fibrina e induce la crescita di tessuto di granulazione per chiudere il difetto.

Secondo il Robert Koch Institute (RKI), il numero di nuovi casi di carcinoma esofageo è raddoppiato dal 1990 in Germania. Le fasce d'età più anziane sono state più colpite. Tenendo conto di un tasso di morbilità e di sopravvivenza invariato, si prevede un ulteriore aumento del tasso di prevalenza a 10 anni nei prossimi anni a 10,500 pazienti.
La pressione negativa per trattare le perdite anastomotiche e le perforazioni nel tratto GI superiore ha recentemente dimostrato di essere il nuovo standard di cura per questi pazienti. [8], [10]
